  • ANY patient with kidney disease
  • Progression of CKD
  • ANY patient with a GFR < 60 m/min
  • ANY patient with an auto-immune disease
  • Blood in the urine
  • Difficult to treat edema
  • Persistent electrolyte abnormalities DESPITE intervention
  • Acid-Base disturbances requiring assistance
  • Recurrent kidney stones
  • Proteinuria/Albuminuria independent of ANY eGFR
  • Acute Kidney Injury
  • Albumin/Creatinine Ratio > 30 mg/g
  • Vasculitis with renal manifestations
  • Hereditary kidney disease
  • Hypertension refractory to treatment with 4+ antihypertensive agents
